The Customer named below authorizes the Company designated above and the financial institution named below (the "Depository") to initiate and complete credit card charges to the credit card account, shown below (the "Account"). The amount of each charge will be the monthly or quarterly (based on service frequency) amount due under Customer's contract with the Company.
